ATF Form 1 SBR

Hey y'all

First, yes I know y'all aren't lawyers so just looking for some opinions. 

Looking at sending in my Form 1 as an individual, and wondering if anyone has done this recently as I have a few questions. Not looking to go the Trust route as I don't want the added cost.

1. CLEO notice - If I read correctly, CLEO sign off is no longer required - only 'notice' is required by sending them a copy of completed app. Do prints and photos need to be on the CLEO app as well, or are these only needed for ATF copy?

 

2. Does an individual sign Box 7, if so, what is "title" for Box 8?

 

3. Bottom of Page 2 - "Signature of Maker"; i assume as an individual that would be myself...?

Tried to Google a lot of this, and many attorneys have web pages on how to fill out, but all the ones I've seen are for going the Trust route. I'm not totally opposed the Trust route, but honestly if it's more than $100 or so, I don't really care to bare the added cost since I'm already in it for $200 tax stamp and then about $700 mods to SBR the rifle.
