Mailing Form 4

  Maybe a dumb question but this whole process has me paranoid.  Do you guys mail your F4 in a separate letter sized envelope inside a USPS priority envelope?  What if anything do you print on the inside envelope?

I fold them up in a letter sized envelope and mail it. I do put tracking but other than that I do not do anything.

I let the dealer take care of it.

 

EDIT:  I just realized you are not using a trust so you need fingerprints and sign off.  I had a trust so I didn't need that (yet).

Thanks guys.  I just noticed that the lady at the Police Dept that did my fingerprint cards did not fill out the 'employer and address' block below her signature.  Should I fill that out or is it OK?

Steve, yes, you'll need to complete that before you send it in.

I mail mine USPS priority so I have a tracking number and know that they received it.  That combined with a check to know when it's cashed is my process.

What goes in that block? Her employer and address or my (applicant) employer and address? The guy that did my fingerprints today told me to put my employer and address in there. I assumed they wanted the employer and address of the person taking my prints.

It should be the person doing your prints employer.

I've listed my (applicants) employer in that block on all my forms and all have been approved no problem.  So, does it matter what goes in that block?

Providing nobody lied , fresh forms are predicted to take at least 15 months. Spring of 2015 is quite accurate. Edited December 8, 2013 by Threeeighty
