ATF  F5330.20  I believe.

 

I sent in my application on the 23rd of Sept, I've been checking my card I used to get charged the $30 for my license. Unfortunlely it hasn't been charged yet as of this morning. 

 

Did I make a mistake by not including this form? I was under the impression it wasn't nessecary.

As of October 1st no applications are being processed.
