I was skiddish about condition 1 at first. Especially with a 1911.
I just didn't understand the mechanics well enough.

UNLOAD AND CLEAR YOUR PISTOL.
REPEAT

Now practice drawing and reholstering your pistol a few thousand times and see what you would have to do to have an AD.

Use a real holster.
Learn to keep your trigger finger indexed and straight.
Know your safety (if you have one).

I did all of the above and find it hard to believe that any one with good safety fundamentals and proper equipment and handling can end up with an AD that isn't at least pointing in a safe direction.

In the military in the combat zone, a negligent discharge is a punishable offense. For an officer, it can really affect a career. Being an officer, the Non-commissioned Officers were always a little leery of an officer with a weapon, even a sharp knife. It never hurt my feelings or hurt my pride to ask an NCO to observe me at the clearing barrel so there were no incidents. After that type of stuff is beat into your head in the service, you don't forget it when you transition to civilian life. The stakes are high on a negligent discharge, so you triple check yourself so there is no incident. Edited November 26, 2012 by QuietDan
