I carried one for years without a holster in my pocket. It fit pretty well in a generic nylon holster IWB but I hated that style of carry, still do. Tried it for a while and it was always an uncomfortable lump. Maks are great but a little heavy, so accomodate for the weight. The gun is awesome but outdated, is how I would put it. Accurate beyond belief (or the 9x18 versions are), reliable all day long, simple and easy to work on, rugged, and one of the best designs ever. The excessive weight, low capacity, large frame, and weakish caliber all point to either getting a smaller 380 or a 9mm in the same sized gun, IMHO, but that is not saying the mak is not a great piece (it is) its just saying that I feel there are better choices due to the small gun trend of the past decade. The gun was made to eat 9x18 so you can shoot the hottest 380s you can find without fear of hurting it -- its almost custom made for buffalo bore which is hard on some of the less rugged 380s. The 9x18 versions are a great bargain gun with cheap ammo and cheap guns, I never understood the 380 version, ammo never was that hard to find.

I did for a while, I have the 9x18. The Makarov doesn't have the prettiest finish on a handgun but it was built to work and it does that very well, I have all the confidence there is in its reliability. I have shot about 500 to 700 rounds out of mine which isn't alot but enough to know how it will function and it NEVER malfunctioned, even with the cheapest/dirtiest Russian ammo. I always felt the .380 and 9x18 was a little under powered but besides that I had no problem carrying a Makarov, maybe my ideas on caliber choice are old fasion these days.
