Mixed Box

I picked up a box of Winchester .38 SPL +p today, along with a box of .357 mag and for some odd reason I pulled out the .38 rounds before storing them. One of them looked odd in the holder. I pulled it out and it was a .40 S&W. (I don't mind since I carry a G23 but I was surprised.) I'm curious to know if anyone else has experienced this type of packaging error? Does it occur often? First time I had ever seen it.

I had a 16ga shell in a 100rnd 12ga value pack from wallyworld a few months back.
