Do you sign the authorization form for the NO-Cost insurance and send it in? $5,000 AD&D and $2500 Armscare...

For some reason, I thought this coverage was automatic. I don't remember having to sign the form in the past.

I never saw anything to sign.
Thats what I was thinking, but, I just got my renewal packet and the insert for the insurance says that you have to sign the form and return it to them to authorize the insurance.

I just got the information packet including my card this evening. There was a brochure offering the $5k accident benefit and $2.5k armscare benefit included with eveything else. The brochure states that it must be completed and mailed within 5 days, which means I will probably wait to the last minute and rush it off to the post office.
