Doctors firearm paperwork

Ok, not sure if this is the right forum for this, but some time ago I had a pdf form that I downloaded that you to give to your doctor if he asks about guns. I seem to have lost that file and cant find it via google. You only use it on docs that are anti. It asked where the doc got his gun training, will his insurance cover you if follow his advice and disarm, etc.

Has anyone seen this around anywhere? I would like to get another copy. I used on a doc a few years ago and it did its job. He never mentioned the topic again.

Why do you need a form?

A doctors office is not a political open forum. It's not a place to discuss shared interest or political differences.

It's a place to discuss ailments and cures. That's all.

It all started with this kind of stuff. Someone decided that if the Dr. was going to council on gun ownership he should show his credentials as a firearms expert.

"Actually my son's pediatrician has a questionnaire on it that ask if you have handguns in your home and if they are stored properly! I mean come on, are people stupid enough to say they have them laying around for the kids to play with. :rolleyes:I thought it was a stupid question for a doctors office!:screwy:"
