Make sure the fore end is racked ALL the way forward? There's a little bit of play in it, cocked or uncocked. As you have to force shells in harder against the spring tension in the last few rounds, one tends to hold BACK on the fore end for leverage. Maybe not it, but that will definitely stop things. edit: I assume you took spring/follower out of mag tube to make sure spring not screwed up or follower out of round or some such? Incidentally, many recommend that you replace the flimsy stock follower with a Wilson combat one, or similar, but that's probably not your immediate problem. - OS

Believe me, there are a bunch of leather makers out there that offer this. Just google: leather magazine + flashlight holster and similar and start researching. Top of my head, TT Gunleather has a nice one. OWB Mag Light combo Holder He makes this one with slots or loops.
